IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

Form 1095-C, Employer Provided Health Insurance Offer and Coverage

is available from the Human Resources Department upon request.

This form is no longer required to be filed with your tax return for employees who worked more than 30 hours per week at any time during January 1, 2024 and December 31, 2024, and/or were enrolled in the Newmeadow, Inc. Health Insurance Plan.

If you would like a copy of this form, please use one of the following options:

Training

To request credit for training please scroll down to the form at the bottom of the page.

**Non-exempt staff must complete training during their regularly scheduled work hours. Administration will coordinate with all non-exempt staff on a time and place to complete the required training.**

EXEMPT STAFF

Once you have completed the required training, please put your training sheet along with the Sexual Harassment Assessment form in the training mailbox in the mailroom/front office.  

NON-EXEMPT HOURLY STAFF

You will be provided coverage to complete the required training.   Melissa Reed/HR will notify non-exempt staff of their scheduled day/time.  Once finished, please put your training sheet along with the Sexual Harassment Assessment form in the training mailbox in the mailroom/front office.

If you are unsure of your status as exempt/non-exempt, please contact HR.

Please only complete the training courses listed on your notice. 

Once you have completed your training course, you will receive a certificate of completion.  Please print your certificate and place it in the training mailbox in the office. Do not forward your certificate via email.

Staff Illness Reporting Form
Newmeadow is required to report to the Department of Health any suspected or confirmed communicable diseases within the school.  Examples include, but are not limited to, COVID, Flu, RSV, etc.  For a full list please refer to form DOH-389 (1/24) pg 2.  Additionally, while individual cases of some diseases are not reportable, a cluster or outbreak of cases of any communicable disease is a reportable event.  A cluster/outbreak includes two (2) or more cases in a classroom or five (5) or more cases throughout the school.  Examples include, but are not limited to, streptococcal sore throat, head lice, impetigo, scabies, pneumonia, etc.  To help us meet the reporting requirement we ask that you please complete the following form.  The reporting of communicable diseases is for tracking purposes only.  You will not be contacted by the Department of Health.  If you have any questions please contact HR.  Thank you.
